Cancellation Policy:
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the doctor or therapists' day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee up to the full price of the appointment(s) scheduled.
1. General
a. The patient’s insurance policy is a contract between the patient and his or her insurance company. However, all charges regardless of the insurance coverage are the patient’s responsibility and the patient is ultimately responsible for any unpaid balances. Acacia Natural Medicine does not bill insurance companies directly.
b. Medicare Policy (for Medicare Part B). If you are a Medicare beneficiary, you understand that the services provided are not deemed medically necessary and/or are considered non-covered services. You will be required to pay in full at the time of visit. No claims will be submitted to Medicare Part B as these are non-covered services.
c. Payment for services rendered is expected at the time of service. We accept cash, HSA/FSA, debit cards and credit cards.
d. If you can’t pay your balance within 30 days, please contact our Billing Office at 480-702-1537. There are several ways you can pay your bill, including possible payment plans and a Billing Office representative will help find the right one for your financial needs. We will also work with you to determine if you are eligible for financial assistance.
2. Past Due Balances
A past due balance is where Acacia Natural Medicine has not received the full patient balance within ninety (90) days. After ninety (90) days as a private pay balance, interest may accrue at the rate of 1.0% per month (12% annual rate) on the unpaid balance at the discretion of the practice. Balances on accounts with payment plans where payments are in compliance with the plan are not considered past due balances.
3. Financial Hardship Determinations
a. For patients, Acacia Natural Medicine may reduce or eliminate the patient’s financial responsibility for medically necessary and appropriate treatment on a case by case basis.
b. Financial hardship determinations are based upon a review of household income, assets and liabilities in relation to current Federal Poverty Income Guidelines. As part of the process, we generally evaluate income levels, net worth, employment status, other financial obligations, the amount and frequency of healthcare bills, and other circumstances.
c. Upon verification of a patient’s financial hardship, the practice may provide a minimum of 15% off and potential maximum of 50% off for eligible patients.
d. The determination of financial hardship is applicable to the current episode of care. To waive or reduce further payments, the patient must again prove financial hardship.
4. Applying for Financial Hardship Assistance
a. The patient or responsible party must identify themselves as someone who needs a financial hardship.
b. We will contact you regarding your application, generally within 5 business days. The representative will inform you of our decision regarding your request for financial assistance and, if applicable, the level of discount for your outstanding Acacia Natural Medicine bill.
